Participation: 
Color, Construct, Create Studios, and its subcontractors do not maintain health insurance for injuries to the participants that may arise out of involvement in this program. By virtue of participation, my child may risk bodily injury and/or other loss, including damage to property. I knowingly and freely assume all such risk for my child. I release and hold harmless Color Construct Create’s staff, it’s representatives, employees, and or/agents from all liabilities and any mishaps that may befall my child, ________________, including, but not limited to, the activities in the classroom and/or outside the classroom facility of Color, Construct, Create Studios Art Classes, except that injury or loss which results from negligence or willful misconduct from one of the individuals or organizations.

Non-Refundable Fees:
Color, Construct, Create Art Class registration documents and fees are due one week prior to the start of each session and are non-refundable. No deferred payment system is available, nor may payments be prorated or a portion of payment fee be transferred to another session. Color, Construct, Create Studios is not responsible for classes missed due to student absences. Color, Construct, Create Studios reserves the right to remove disruptive influences from their class. No refund will be given in the event of a dismissal. Failure to submit registration documents and fees one week prior to the start of the session signifies the release of the student’s participation for that session.

Transportation:
By registering your child for participation in Color, Construct, Create Art Classes, you agree to adhere by the start/end times of the program. I understand that we the family, are responsible for arranging all transportation for my child to and from Color, Construct, Create Art Classes. There will be a $10 charge for every 15 minutes thereafter.

Alternate Pick-Up People:
The protection of your child is of the utmost importance. If your contact is not listed to pick up said child, your child will not be allowed to leave premises. No permission-no pick up!

Special Medical Needs and Medical Consent with Liability Release:
Color, Construct, Create Studios and it’s owners/staff will first use reasonable efforts to contact the parent/s and/or guardian in the event of a minor incident. In the event that the parent or legal guardian is not available in a case of an emergency, I hereby grant Kristin Cockrell and Kelly Marshall of Color, Construct, Create Studios, and its staff, my permission to secure emergency medical treatment for my child. I understand that medical information and personal data will only be used when necessary to protect a child’s well being.

Food Allergies and Allergies Notification, Acknowledgment, and Release of Liability:
We create art with various forms of media including but not limited to food products, paint, wood, metal, medium gels and pastes as well as many more. An environment free of allergens, including but not limited to above stated CANNOT be guaranteed at Color, Construct, Create Studios. While reasonable efforts will be made to avoid all contacts with hazardous products, the Released Parties CANNOT guarantee that any product is free of all traces of any particular allergen, or that the Participant will not come into contact with any allergens while at Color, Construct, Create Studios. The undersigned acknowledges and agrees that he/she is aware of such risks and that participation in the Program could expose the Participant to food, and various products and mixed media supplies through involvement of said activities, and persons that may result in exposure to allergens and injury and in that regard and assuming such risks, the undersigned hereby fully releases and discharges the Released Parties from any and all liability, and/or responsibility to the Participant, the undersigned, or any third party for injuries to the Participant/and or any direct, indirect punitive, incidental, or any damages that arise out of/or relate to Participant’s participation in the program Color, Construct, Create Art Classes and /or exposure to food, paint, wood, metal products or various unknown allergens above mentioned.
